Adult Checklist

If you check one or more of the following, it is highly recommended that you have a comprehensive eye test. An online appointment request facility is available.

It has been more than two years since your last eye examination
You are diabetic
You have high blood pressure
You are over 40 years of age
You have a relative with glaucoma
You have a close relative with diabetes
You spend more than two hours per day on computers
You get headaches
You have problems seeing street signs
You often have itchy eyes
You often have red eyes
You often have watery eyes
You see "floaters" or flashing lights in your vision
You sometimes get double vision
You have problems seeing small print
You notice your distance vision is blurry after you have done a lot of close work
You have sore or tired eyes
Your eyes feel fatigued when reading

Contact lenses can be an alternative to glasses in providing clear vision. You may be suitable to wear contact lenses if any of the following apply to you:

There are times in the week when you would rather not wear glasses
You play contact sports
You would like to wear non-prescription sunglasses
You would like your peripheral vision to be unrestricted by the size of your frame
You would rather not wear glasses when you are "dressed up" for special occasions
Glasses are hard to keep clean in your work environment
You find glasses heavy or uncomfortable
Your children like to pull your glasses off your face
Glasses are not practical for your work

Checklist for Children

A tick against more than one of the following could indicate that a vision problem is present. A comprehensive eye test is advised.

APPEARANCE OF EYES

One eye turned in or out
Frequent blinking
Squints or screws up eyes
Red eyes or lids, crusting on lids
Frequent styes or infections
Excessive eye movements
Excessive watering of eyes or light sensitivity

COMPLAINTS

Headaches
Difficulty seeing clearly at distance
Blurring of vision while reading or writing
Seeing double
Eye burning or itching during or after close work

BEHAVIOUR

Holds book very close
Avoids close work
Loses place when reading, skips lines
Omits words or makes errors when reading or copying
Closes or covers one eye when reading or working at near
Confuses similar words, fails to recognise same word in different context
Has a short attention span when reading or writing
Has poor or unusual sitting posture when reading
Tilts head excessively to one side, up or down
Makes excessive head movements when reading
Squints or frowns to see blackboard clearly
Rubs eyes frequently
Thrusts head forward to see distant objects
Has obvious tendency to favour one eye
Is nervous, irritable, tense or restless after maintaining visual concentration
Makes errors in copying at near or from distance to near

Diabetes Risk Assessment Checklist

Optometrists play a fundamental role in detecting eye damage from diabetes in its early stages - problems such as glaucoma, cataracts and retina damage. Regular
eye examinations are vital for the detection and management of problems associated with diabetes.

If you tick one or more of the following boxes, you could be at risk of diabetes:

I am over 50 and have high blood pressure
I am over 50 and am overweight
I am over 50 and one or more members of my family has/had diabetes
I am over 65
I have heart disease or have had a heart attack
I had high blood sugar levels while I was pregnant (gestational diabetes)
I have had a borderline high blood sugar test
I have Polycystic Ovary Syndrome and am overweight
I am over 35 and am an Aboriginal or Torres Strait Islander
I am over 35 and am a Pacific Islander from a Chinese cultural background or from the Indian subcontinent
